Fix Known Bugs

1. The du command is disabled. The df command works only on / or the entire volume.

2. Index updates cause an exclusive lock to be obtained on the index object (thereby locking the entire index).

3. Application executables are too large. We should be able to re-structure the libraries to cut down the sizes. Some of the reasons might have to do with templates.

4. The legitimacy of logical OIDs pre-allocated by the object cache is not checked.

5. Bulk-loading an empty index (using sorted streams) causes an assertion failure.

6. An assertion can fail if a zero-sized object is created with a hint indicating that it will be large (this can only be done through the SSM API).
